Access this screen by selecting Tool #661 Print Member Certificate Form.

This screen is used to print a standard CU*BASE laser certificate form. Forms are credit union-defined. For information about configuring and printing CD forms, refer to the User-Defined Laser Certificate Forms booklet.

NOTE: If it has been configured in the membership designation, the DBA label will appear on the certificate instead of the member’s or organization’s name.
