Screen Overview
This is the twelfth and final screen in a series of screens used to “capture” the data required to print on the Closing Disclosure.The data entered on this screen is printed on page 5 of the Closing Disclosure. There are 3 columns to be completed: Lender, Mortgage Broker and Settlement Agent.

Enter the appropriate address. There are 2 lines available to enter the address |

NMLS ID |
Enter the NMLS number assigned |

State / License ID |
If there is not an NMLS number assigned, enter the state code and the state license number assigned |

Contact |
Enter the appropriate contact name |

NMLS ID |
Enter the NMLS number assigned to the contact name |

State / License ID |
If the person listed as the contact does not have an NMLS number assigned, enter the state code and license number assigned |

Enter the appropriate phone number (including area code) |
