Records will be sorted by account base. The following fields will be displayed in this Query:

• Branch Number

• Account Base

• Closed Sequence Number

• Member's Last Name, First Name, and Middle Initial

• Social Security/TIN

• Address Lines 1 and 2

• City, State, ZIP Code

• Area Code, Phone Number

• Sex

• Birth Month, Birth Day, Birth Year

• Opened Month, Opened Day, Opened Year

• Closed Month, Closed Day, Closed Year

• Years as Member (no partials)
