This window appears after using Add Person (F6) on the Currency Transaction Reports (CTRs) screen.

This window starts the process of entering a SSN/TIN of an additional joint owner, if not coming from the Teller system.  Enter a SSN and transaction date and use Enter to proceed to the second CTR Forms fill-in screen.
